Graduate and Professional Admissions
Application Checklist
You will need to have several types of information available as you complete the Graduate College application for admission. Below is a quick checklist to see if you have everything you need before you get started. For more detailed information, please see the application instructions.
Personal Information

If you attended the University of Illinois before, it is helpful to have your UIN, and you need to know the last term you attended.

You need to know where and when you were born.
_____
Go to www.grad.uiuc.edu/admissions/apply/citizenship.asp if you are not sure of your citizenship status.

If you are a domestic student, you will be asked to provide your SSN (this information is not required, but helps speed up the application process).
_____
If you are a resident alien, you will need resident alien number. In addition, your lawful Permanent Resident card ('Green Card'), Letter of Approval Notice from USCIS, or a copy of the Personal Information page from your passport as well as your Permanent Resident stamp should be sent to your proposed program of study office.

If you are an international student, you will need to know passport and current visa status information.

What Graduate Program of Study are You Applying To?
You will need to know:

The proposed program's contact information

The degree name

The deadline for applying to the program

Your chosen field of specialization within that department

Visit the departmental Web page to see if the program requires any additional information or samples of your work (writing sample, portfolio, etc.)

Find out what exam scores are required by the program (GRE, MCAT, TOEFL, etc.). You will need to be sure that official scores are sent to the University of Illinois from the testing agency. (For ETS our school code is 1836.)
Educational Institution Information
Filling out the application will be much easier if you have a copy of your transcript from each institute of higher education you have attended. You will need to know:

Overall and major GPAs

Major area and name of any completed minors

Send an official transcript from each of these institutions, to each program of study office, for each application that you submit. Contact each of your previous institutions to find out how you can request that they send official documents. International applicants have special instructions; please see the Transcripts/Educational Credentials sections within the application instructions for important information.
Test Information

You can enter test scores and other testing information into the application if you have the score sheets when you fill out the application.
Statements

The application will ask you to upload an up-to-date resume (max. 750 words)

Most departments require a statement of purpose or personal statement (max 1000 words)
Recommendations
You will need the following information for at least three references:

Credit Card Information

You will need to have a credit card in order to submit the application fee. If you are a University of Illinois employee, or a fee waiver recipient (McNair Scholar, Project 1000, CIC Free App) be sure to choose the correct waiver type in the drop down on the application.
