Departments are required to submit the Graduate College Rating Form for Fellowships and Traineeships in the following situations:
- to nominate a particular student in a campus-wide fellowship competition
- to process all fellowship and traineeship appointments
- to bill tuition and fees to an account

The Rating Form is available as a PDF file which can be downloaded. Please use the following instructions to assist in the completion of the Rating Form.
The requirements for completing the rating sheet vary according to the type of fellowship appointment, as listed below:
- Graduate College fellowship and grant competitions: Complete the rank section as well as sections 1 - 10. Attach any other material required for the competition. This applies to Illinois Distinguished Fellowship, Graduate College Fellowship for Underrepresented Students, University Fellowship, Dissertation Completion, and Dissertation Travel Grants.
- Departmental fellowships: Complete sections 1 - 8. Attach copy of student graduate application.
- Training grants: Complete sections 1 - 8 and attach agency Notification of Award.
Sections
- For fellowship submissions list UIN and student's name.
- a. State the student's department.
- In the case of students in the Medical Scholars Program (MSP), please list both MSP and the other academic program.
- In the case of MSP students, please list the PhD-granting department.
- List the student's current mailing address and permanent address. If both addresses are the same, fill in the mailing address and indicate "same as above" on permanent address. Indicate International Status, Race/Ethnicity, Gender, Date of Birth, and Country of Origin.
- Provide the appropriate fellowship name.
- If rating form is for tuition and fee waiver, select term(s) and the year.
- Provide C-FOAP title, C-FOAP, time sheet org, dates of tenure, monthly stipend, and total stipend of tenure.
- If appointment is for a traineeship, please attach a copy of the completed agency trainee form.
- Waivers and Fees:
- If fellowship is for $3,000/semester or more and tuition is not to be charged to a grant, check the "waive" box.
- If fellowship is in addition to an assistantship between 25 and 67%, check the "Asstshp wvr" and indicate on the line the percentage of the assistantship.
- If fellowship tuition and fee charges are to be billed to a grant, check "Bill to stipend C-FOAP."
- If the payment of other fees (balance of health insurance, general, and transportation) are to be billed to a grant, check "Bill to stipend C-FOAP."
- If the other fees should be charged to a departmental C-FOAP, check "other C-FOAP" and provide the C-FOAP in the space provided.
- If a grant or departmental C-FOAP is not going to pay for the other fees, check "no pay." This means that the student will be billed for these fees.
- List the schools student has attended, dates of attendance, GPA, number of credits used in computation of GPA, and degree awarded from institution.
- List all appointments the student has for the current academic year.
- Check whether the student is currently enrolled full-time. Full-time enrollment is required for Fellows except in cases where a lower enrollment can be justified by the academic program and is approved by the Graduate College.
- List GRE, MCAT, or GMAT scores, if required for admission into department. If not required, write "Not Applicable" in this section.
- List TOEFL Score if applicable.
- Provide any additional relevant comments.
- Approval: Department Head, Chair, or other authorized person must date and sign to indicate approval.
